Please make sure to read through the application information for incoming exchange students before completing these forms:

• Conduct Statement
• Financial Verification: Include copies of a recent bank statement and/or a letter of support if those funds are in a name other than the student’s. Scholarships, grants, or government funds require a letter of offer which states the amount being disbursed. Students should be advised that the US Embassy will also require proof of funding when they go for their visa interview. *See Estimated Expenses
• Student Health Insurance Forms (you will need to complete the following forms)
• USF Immunization form
• Insurance Agreement Form
• Housing
• Meal Plan Forms
